my server restarted and this was the errors that were in the event log file

error: Failed to open/read local data from file/application

warning: Can't read pathname for load map: Input/output error.

warning: .dynamic section for "/servers/theadultgungame/orangebox/cstrike/addons/sourcemod/extensions/game.cstrike.ext.2.ep2v.so" is not at the expected address (wrong library or version mismatch?)

Well, by reading the error - something in the sourcemod/extensions folder is not at the expected address.
